Eerste controles / First checkups

Na ruim een week was ik weer thuis en dat ging eigenlijk best goed.
Wat me wel erg tegenviel was, hoe weinig ik zelf kon. Ik lag veel, had een heerlijke relax feautuil in de woonkamer en een verstelbaar bed op mijn slaapkamer. Mijn moeder heeft alle verzorging gedaan en dat ging echt super. Elke dag heeft ze mij geholpen met douchen, wat al een grote taak op zichzelf was. Zij heeft ook mijn verbanden elke dag verwisseld. Ik kan wel zeggen dat ik echt de beste hulp die je kunt wensen hebt gekregen.

ENGLISH: After more than a week I went back home and I did pretty well. What I didn't like to much, was how much I couldn't to myself. She took care of me. Helped me showering, which was one big task on itself. She put on the bandages every day and did that very well! I can say that I think I had the best help I could wish for.

Eerste controle en urethrogram / First checkup and urethrogram

9 december was dan de dag dat ik voor de eerste controle naar het ziekenhuis moest. Samen met mijn vriend vertrok ik naar Amsterdam, naar de afdeling radiologie. Ik had nog steeds de twee catheters en liep nog wel wat langzaam. De catheters begonnen af en toe al wat te irriteren. Ik werd geroepen door de medewerker en mocht me uitkleden, shirt mocht ik aanhouden. Eenmaal binnen was het de bedoeling dat ik op een tafel ging liggen. De radioloog had de taak op mijn catheter, die door de plasbuis liep, eruit te halen. Een spannend moment, vond ik. Het gebeurde ook erg langzaam, wat het niet fijner maakte. Even kort pijnlijk, maar goed te doen. Daarna kreeg ik een kussen in mijn zij, zodat ik half op mijn zij lag.

On december 9 I had to go to the hospital for the first post op checkup. With my boyfriend I went to Amsterdam, the radiology department. I still had the two catheters and walked a bit slow. The catheters were a bit irritating already at this point. I got called in and had to undress. I had to lay down on a table. The doctor pulled out the catheter (not the suprapubic). Kind of exciting moment. He pulled it out very slowly, not to damage anything. It hurted a bit, but wasn't to bad really. After that I had to lay on one side, with a cushion in my back.

Zo kreeg de radioloog de beste foto's van mijn plasbuis te zien. Dus ik lag, half op mijn zij en kreeg een fles waar ik in moest plassen. Via de suprapubisch catheter spoten ze contrastvloeistof in mijn blaas (voel je niets van!) en dat moest ik dan weer uitplassen. Op het moment dat ik plaste, moest ik het aangeven en werden de foto's gemaakt van de vloeistof door de nieuw gemaakte plasbuis. Een goede manier om vernauwingen en fistels te ontdekken!

This way, on my side, they could shoot the best pictures of the new urethra. So I laid there on my side and got a sort of bottle to pee in. They inserted some contrast media in my blatter through the suprapubic catheter (you don't feel anything from this). Then I had to pee this out through my penis, in the bottle. While peeing, I had to tell, so that they could make pictures of the fluid in the urethra.

Echter, ik kreeg het niet voor elkaar. Ik had wel wat aandrag, want er was immers vloeistof ingespoten. Maar ik denk dat ik niet kon plassen door een aantal redenen. Ten eerste, er kijken 3 mensen naar je. Afwachtend op je plas. Ten tweede, liggend plassen is ook niet natuurlijk en daar moet je ook een knop voor omzetten. Ten derde; dit was mijn eerste plas door mijn piemel! Ging het pijn doen?! Waar ging het overal uitkomen?! Ging het er uberhaupt uitkomen?! Met name die laatste reden vond ik onbewust denk ik too much. En dus werd ik met tafel (en rontgenapparaat) gekanteld, zodat ik tegen het bed aanstond. Huppekee, 90 graden omgedraaid. Ogen dicht, nergens op letten en gaan... Het lukte weer niet. Ik probeerde het zo'n 15 minuten toen ze voorstelden eerst een andere patient te doen.

But... I couldn't do it. I do had the urge a bit, since they inserted the fluid. But I think I just couldn't do it, because; 1. Three people watched me. Second; peeing while laying down is also not a nutural thing to do for me. And third; this was my first time peeing through the penis! Was it gonna hurt?! Where was it gonna come out?! Wás it even coming out at all?!
Especially that last reason was too much I think. I just couldn't switch that button. And so I was, while laying on the table, rotated 90 degrees. Now I was standing up, leaning against the table. Maybe it would help, standing up. I closes my eyes and tried really hard... I didn't succeed. I tried for like 15 minutes, untill they suggested to help another patient first.


In de tussentijd dronk ik wel 6 glazen water in 5 minuten. Dat mocht, want de contrastvloeistof mocht wel wat gemengd worden met water. Ik had nu toch wat meer aandrang toen ik weer aan de beurt was. Hetzelfde ritueel en weer 3 paar ogen die 10 minuten naar me keken. Het lukte niet. Helaas. Dit was geen ramp en ik mocht onverrichter zaken naar mijn volgende afspraak op de poli plastische chirurgie. Daar had ik rond die tijd een afspraak met Bouman zijn assistent. Deze aardige dame bekeek mijn wonden en was zeer tevreden. Zij was er ook bij tijdens mijn operatie en heeft me voor de operatie ook nog een paar keer gezien. Ze vond het jammer dat het plassen niet was gelukt, omdat bij mij volgens haar alles volgens het boekje ging. maar niets aan te doen. Nadat ik me weer had aangekleed, moest ik zó nodig naar de wc, dat ik op de poli zelf ging. Ik ging zitten, liet het los en... het liep eruit! Ik plaste door mijn piemel! Ik bekeek alles natuurlijk grondig en was helemaal blij. Nu moest ik thuis telkens door de piemel plassen en daarna het dopje van de suprapubisch afhalen. Zo kon ik de blaas dan helemaal legen, zodat er geen retentie overbleef. Bij minder dan 50 ml retentie mag de catheter eruit. Ik was allang blij dat ik altijd nog een 'reserve' uitgang had.

While waiting, I drank like 6 glasses of water in 5 minutes. I had more urge now, when it was my turn again. The same ritual now and again those 3 people watching me for like 10 minutes. And I still could not do it, too bad. It was not a disaster that I couldn't and so I could go to my next appointment in the hosital. I had an appointment with dr. Bouman his assistant. This nice lady checked my wounds and was very happy with how it looked. She was there at the surgery and I have seen her a few times before the surgery as well. Everything went so well, so she was a bit disappointed of the fact that the peeing didn't go that well. After the checkup I had to go to the toilet so bad, that I went on the unit plastic surgery itself. I sat down, let go and.... there it went! I was peeing through my penis! I checked it out and was of course so happy to see it went well! Now I had to pee this way all the time at home. And after doing so, I had to take of the cap of the suprapubic catheter to empty the blatter. Doing so, I made sure that I didn't have any retention. At less than 50 ml. retention they would take out the catheter. I was happy that I still had this 'second exit', just in case.

Een paar dagen later, op 14 december had ik nog een checkup bij dr. Buncamper. Ook hij was heel tevreden. De suprapubisch catheter bleef nog even zitten. Op 22 december kwam ik voor de derde controle. Dit keer was ik bij een arts assistent van Bouman die ik nog nooit had gezien. Hij vertelde dat de catheter nog steeds bleef zitten. Bouman kwam later even kijken en besloot dat de catheter er wel uit mocht. Een verpleegkundige deed dit. Het was haar eerste keer dat ze dit deed. Ik kan wel zeggen dat dit het meest pijnlijke was, van alle dingen in het operatie proces. Bouman vertelde ook dat, als de plasstraal minder zou worden, ik heb echt aan hem moest doorgeven. En een dag later was het al zover. Ik plaste steeds minder. Ik moest wel genoeg, maar het kwam er met een steeds dunnere straal uit. Er kwam een vernauwing in mijn plasbuis en ik belde het VU. Dr. Ozer, Bouman zijn arts assistent (de aardige dame die me al vaker had gezien), deelde na overleg met Bouman mee, dat ik naar Amsterdam moest komen. Ik reed er direct heen en zij leerde me daar te dilateren met een catheter. Met een catheter, maatje 16, moest ik elke dag gaan dilateren om de straal goed te houden. Ik moest zelf de catheter zo ongeveer 15 cm naar binnen duwen. Ik was erg zenuwachtig om dit te doen, maar voelde er niets van.

A few days later, I had a second checkup at dr. Buncamper. He was also very happy with the results. The suprapubic catheter still couldn't go out. On december 22th I came for the third checkup. This time I met another assistent of the doctor. I have never seen him before and he told me that he wouldn’t take the suprapubic catheter out yet. After Bouman came in and checked up, he decided that he would a nurse have to takei t out. It was her first time and I can honestly say that pulling the catheter out, was the most painful thing was during this process. Bouman also told me, that if I have any problems with urinating, I should let him know immediately. And a day later, I had to call him already. I peed less and the urinating was quite difficult. I came out with a less strong stream(?) than before. I think I was developing a stricture in my urethra and so I called the VU hospital. Dr. Ozer, Bouman his assistent (the nice lady I met before) told me, after calling dr. Bouman, that I had to come to Amsterdam again. I went there immediately and she tauught me how to dilate with a catheter. With a catheter, size 16, I have to dilate the urethra every day to keep it open. I have to push it in for like 15 centimeters. I was very nervous doing this, and was happy to find out that it didn’t hurt at all.
