Na de correctie-operatie / After the correcting surgery

Afgelopen 28 juni ben ik weer geopereerd. Ik ging onder het mes om de fistel te sluiten, de vernauwing weg te halen en om nog wat weefsel van het topje van de penis te verwijderen. Niets groots. Ik hoefde dan ook maar 1 nachtje te blijven. Rond 7 uur in de morgen kwam ik aan op de afdeling, erg vroeg voor mijn doen. Al direct mocht ik me omkleden, ik zou rond half 8 al opgehaald worden. Alles ging goed en echt nerveus was ik niet. Misschien kan het door het pilletje dat ik zo kalm was. Of misschien was het wel om het feit dat ik nog niet zolang geleden ook geopereerd was. Ik wist immers nog precies hoe het ging. Het volgende dat ik weet is dat ik wakker werd. Direct keek ik onder de deken en ik zag inderdaad een buisje uit de penis komen. Maar hij kwam maar een aantal centimeter uit de penis en was niet aangesloten op een zak. Ik vroeg de verpleger wat de bedoeling was, aangezien ik toch echt wel een catheter zou krijgen? Hij weet het niet, maar de dokter zou zelf later alles uitleggen. Ik ben te moe om er echt iets van te begrijpen en na een uurtje in slaap vallen en diezelfde vraag 10 keer te stellen, word ik terug gebracht naar de afdeling.

ENGLISH: I had the surgery for fixing the fistula and stricture on june 28th. They also had some tissue to remove from the top of my penis. So nothing big really, I only had to stay for 1 night. I got to the hospital at 7 o'clock in the morning, so that was super early. I had to change to the surgery shirt immediatly as it was my turn at 7.30 already. Everything went fine and I wasn't really nervous. Maybe it was the little pill I got to stay calm. Or maybe it was the fact that I only had surgery a few months ago. The next thing I know is when I wake up. I check under my blanket and see a tube coming out of the tip of the penis, but it's not connected to a bag or something. It's just the tube in there. So I ask the nurse what happened. Because I knew for sure that I would get a catheter. But he doesn't know, the doctor will explain everything later. I'm too tired to really care and after an hour and having that same question asked for about ten times, they take me to my department.

Eenmaal terug op de afdeling, voel ik al gauw de drang om te plassen, aangezien ik dus geen catheter heb. De narcose zit er nog flink in en ik weet nog steeds niet wat er nu aan de hand is. Maar de aandrang wordt zoveel dat ik toch echt naar de wc moet. Hij geeft me een mannen-urinaal, zodat ik op de rand van het bed kan zitten plassen. Ik vertrouw op de verpleger die me de urinaal gaf. Nog steeds ben ik niet zeker van hoe dit zal gaan, maar ik laat het maar lopen. En daar gaat het met een mooie ferme straal! Maar niet in de urinaal, alles loopt tussen mijn benen en klettert op de grond. Even raak ik kort in paniek. Maar dan bedenk ik me ineens; wacht, ik heb weleens gehoord van zoiets als een 'achterdeur'. Dat betekent dat ze de oude en nieuwe plasbuis weer los van elkaar hebben gemaakt. Ze maken een klein gaatje achter je scrotum en daar eindigt je oude plasbuis. Je plast dus vanuit dat gaatje. Maar, ik plas tenminste. En dat zonder dilateren of hard te hoeven drukken. En het komt er hard en met een flinke straal uit, zonder enige pijn. En dat voelt goddelijk. Na zo'n lange tijd niet goed te kunnen plassen, voelt dit fantastisch.

Since I don't have a catheter, I feel that I need to go to the toilet. I'm still a bit tired and dull from surgery and I'm still not sure what has happened. What I do know is that I have to pee so I ask the nurse what to do. He handed me a bedside urinal, the kind for men. So I sit on the bedside and am ready to start peeing in there... I trust the nurse, but am still not sure how this is going to work so I just let go. And there it goes. All down my legs and onto the floor. Nothing's coming out of my penis. For just a few seconds I'm panicking. But then I start thinking; wait, I heared something like this before. Do I have a 'backdoor' now? That is when they disconnected the old and new urethra. They make a little hole right behind your scrotum and that's where you pee through. But, I am peeing. Without having to press it out. And it's coming out fast and with such a big stream(?), without any pain. It feels wonderful I have to say. After such a long time not peeing that well, this feels wonderful.

Mijn verwachting van de 'achterdeur' waren inderdaad waar. Na een paar uur en al wat po-bezoekjes later, komt de dokter dan langs. Het spijt hem dat het niet gelukt is zoals ze hadden gedacht. De vernauwing was te heftig om doorheen te komen, dus moesten ze de penis bij de fistel openmaken, van buitenaf. De vernauwing had een lengte van 1 cm en dat was vrij groot, vertelde hij me. Toen ze dit gefixt hadden, kwamen ze nog een andere vernauwing tegen. Ze besloten de hele boel open te laten en me tijdelijk te laten plassen door de achterdeur. Daarom heb ik nu het buisje in mijn penis. Aan het begin, bij de eikel komt hij de penis in. Daar waar de fistel zat, aan de basis komt het buisje eruit, waarna het direct weer naar binnen gaat. Dan komt het buisje er aan de achterkant, achter het scrotum er weer uit. Daar zit ook die achterdeur. Ik urineer nu langs het buisje heen.

My feelings about the 'backdoor' were right. After a few hours and some trips to the bathroom the doctor visits me. He's sorry things didn't work out the way me and they wanted it to. The stricture was too tight to get through, so they had to open the penis at the point where the fistula is. The stricture also had a length of 1 cm. And he told me that's quite large. Then, they found another stricture further into the urethra. They decided to keep this all open and then make me pee through that backdoor for now. This is why I had this tube. It's coming in on the tip of the penis. Coming out of the penis and the base, where the fistula was. Then it goes straight back in again and coming out at the back of my scrotum. Where the backdoor is as well.
